What form from the dmv a peace officer must fill-out to exempt thier personal vehicle. For example, a off-duty peace officer is driving his personal vehicle and a B/W(black and white) patrol vehicle runs the vehicle's plate. Information comes back with name ,d.o.b ,ect ,but no home address. The home address shows the law enforcement department they work for. Thanks for the info.

Its the DMV form of confidentialty, and you get them from your dept and have to be signed by a dept rep.

And it doesn't exempt your vehicle, you just have your address made private.

Dispatchers are allowed to submit the form as well.

DMV Form 57 (rev) - Request for Confidentiatlity. Must be signed by your Chief, Sheriff or authorized Departmental representative.

It also doesn't display your dob. just your name and the department its 10-35 to.
