Hard drug use, excessive drug use (hard or "soft"), deception, bad employment history, criminal record, credit, driving record, academic record, financial status, gang affiliation, deception, late to appointments, attitude (entitlement, arrogance, confrontational), did I say deception?

I can tell you that as long as it is done in places where it is legal and the rest of your background is clean, it is not an automatic DQ

Not being honest with your employment app and background papers will do it. As stated, dishonesty is a big one, as it goes to your credibility to testify in court.
