555 form

Anybody have a MS Word or fillable .pdf version of the 555? Sometimes it's easier to just use one of those applications than to have to find a computer that has CARS installed.

I can PM a work e-mail address for it to be forwarded to as an attachment.

Ours are in OmniForm, not MS Word. If you have that program at your agency I can send it to you. I have the 555 and the 555 additional info forms in OmniForm.

I found this posted on the internet; it is from Capitola PD. Perhaps you can capture it and modify it to your needs. It is in pdf.
