NYPD APD forms -I made pdf

I made an fillable pdf file for APD form 16, so you wont have to find a typewriter. I couldn't find somebody who made one already. So here is a link. It is 2 pdf files, one for each side.

I'll see if I can combine them into one page. I plan on making fillable PDFs for all the forms.(unless somebody else has done this already!!)

I'm working on it. I'm gonna try to put all the forms not available for download on NYPD website into fillable pdf.
