Which forms we must have when we meet with our investigator other than the APD-5?

There are so many forms I don't know which one should I bring to the investigator when meeting with them.

all of them. if it was giving to you at the medicalhave it all typed up and bring it all. it says only a few need to be typed, i did all so there were no problems.

I agree with NYC1219. TYPE all the forms up. That's the very first thing my investigator told me to do. She said I know the paperwork says on certain papers have to be typed, but I want them all typed.
